Bronson Advanced Cardiac Health (BACH)
Heart disease can take many forms and affect people of all ages. BACH providers specialize in evaluating, diagnosing, and treating adult heart disease.
Details about referral priority:
Emergent referrals need to be sent to the emergency room
Urgent referrals require a phone call to the office to speak with the physician on call
Routine referrals do not require a phone call or message to the office or physician on call; The patient will be seen within 1 month
Things to know when referring to BACH:
All referrals are evaluated and risk stratified based on symptoms and potential disease severity by our clinical staff and providers to determine priority.
Having required documentation and/or cardiac testing completed prior to cardiology appointment will facilitate/speed up referral appointment request. Consider share point site guidelines to assist in testing decisions.
Cardiac testing that is deemed high-risk will likely result in a phone/Epic message to the ordering provider to facilitate urgent referral/evaluation if at all possible.
